They do not come with blank envelopes. You would need to give us a call and the envelopes would be added to your order. However, we don't offer blank envelopes for the card sized 3.5" x 5". We have blank envelopes for all the other sizes.
Yes, you can have a plain uncoated back so it it easy to write on. Please select 14 Pt High Gloss (UV) Front, Uncoated Back as the Paper choice.
The way our pricing would work is these would be six different designs as we charge per design per quantity.

Definitely, most of our customers do. Just make sure your artwork has at least 300 Dpi resolution and uses a CMYK color format when you submit. There are templates near the bottom of the product page which can guide you in terms of the size, safe zone and bleed requirements. You can also use the "Create Your Design Online" option where you can type in your own text and insert your own images.
